Current and former Kentucky state government employees and applicants to positions in Kentucky state government may request a copy of their official personnel file as maintained by the Kentucky Personnel Cabinet pursuant to KRS 18A.020 by completing this form. The Office of Legal Services will process all requests. Click on the box to the left to submit your request​. ​

Current and former Kentucky state government employees may request removal of a written reprimand from their official personnel file as maintained by the Kentucky Personnel Cabinet pursuant to 101 KAR 1:335 Section 5 by completing this form. The Office of Legal Services will process all requests on behalf of the Kentucky Personnel Cabinet Secretary. Click on the box to the left to submit your request.
