Executive Safety Advisory Committee (ESAC) Safety Award
In order to promote the safety and health of all Commonwealth employees, the Executive Safety Advisory Committee has instituted the ESAC Award. This award will recognize those Cabinets, Departments, Divisions or equivalent of the Executive Branch that achieve 12 months of work without experiencing a lost time injury. The award is designed to support all programs that intend to reduce occupational injuries.
Winning cabinets will be awarded a plaque, departments/divisions will receive certificates of recognition. 2015 awards will be presented at the ESAC meeting in June during 2016 National Safety Month.

Application Process
To ensure that the requirements are fair and equal to all applicants, approval of the award is based on information contained in the employer's injury and illness records maintained and verified in accordance with the recordkeeping requirements of the Kentucky Occupational Safety and Health Program. All entries must be received no later than May 31.
Cabinets/Departments/Divisions must submit the following with appropriate documentation:
Application
OSHA Form 300A Summary
Complete Documentation of Total Hours Worked (this is found on the log)
Please note: application, logs, and documentation must be signed.
